For making an appointment you can reach us by phone on weekdays from 08.30 - 17.00, fill in an appointment request or send a call back request .
For other questions we kindly request you to contact the secretariat, you will be automatically connected to our appointment office in Leidschendam.

Appointments cancelled less than 24 hours in advance will be charged to the patient.
